The maximum amount of each claim for “vascular intervention treatments for aortic diseases or aortic aneurysms” and “early-stage malignancy” for the same insured individual under LFH Life’s policy is MOP/HKD 400,000 (subject to the currency specified in the policy schedule).






(a)

No major illness benefit, minor illness benefit, or juvenile illness benefit shall be payable under this policy for any major illness, minor illness, or juvenile illness first diagnosed, or for the related symptoms or conditions first appearing or occurring within ninety (90) days from the policy issue date, the date of endorsement, or the effective date of the last reinstatement, whichever is the latest. Item(a) does not apply to any major illness, minor illness or juvenile illness caused by an accident.




(b)

No major illness benefit, minor illness benefit, or juvenile illness benefit shall be payable under this policy for any major illness, minor illness, or juvenile illness resulting from, or related to, or caused directly or indirectly, wholly or partly, by any of the following:


(i)

Any pre-existing condition;


(ii)

Any congenital deformity or abnormality, infertility or sterilisation;


(iii)

The taking of drugs not prescribed by a physician, alcohol abuse or the taking of poison;


(iv)

Any major illness, minor illness or juvenile illness diagnosed after the insured’s death;


(v)

Any Human Immunodeficiency Virus (HIV) disease or infection and/or HIV-related disease including Acquired Immune Deficiency Syndrome (AIDS) and/or any mutation, derivation or variation thereof, except for HIV infection due to blood transfusion or occupational exposure, as specified under the Payment of Benefits. Under this Policy, the definition of AIDS shall be that used by the World Health Organization in 1987, and as may be revised by the World Health Organization from time to time;


(vi)

Ionising radiation or pollution resulting from the radioactivity of nuclear waste produced by nuclear fission, nuclear fusion, the use or burning of nuclear fuel, or the use of nuclear weapon materials;


(vii)

War or hostilities (whether declared or not), civil commotion, rebellion, revolution, riot, strike, or any terrorist or warlike activity;


(viii)

Participation in any armed force or peacekeeping activity;


(ix)

An act of any person, acting on their own or on behalf of or in connection with any group or organisation, to influence by force any group, corporation or government by terrorism, kidnapping or attempted kidnapping, attack, assault, or any other violent means;


(x)

Any intentional self-inflicted act;


(xi)

Violation or attempted violation of the law, resistance to arrest, or participation in any brawl or affray;


(xii)

Any professional sport, competitive event, underwater activity involving the use of breathing apparatus, aerial activity (including bungee-jumping, hang-gliding, ballooning, parachuting, and sky-diving) except as a crew member or a fare-paying passenger in a licensed, regular commercial passenger aircraft, or any hazardous activity or sport unless agreed by special endorsement.

The Policy Owner is subject to the credit risk of LFH Life. If the Policy Owner discontinues and/or surrenders the insurance plan during the early years of the policy, the benefit they receive may be considerably less than the amount of premiums they have paid.

LFH Life reserves the right to decide whether to accept or reject any application for the Plan and its optional benefits (if any) according to the information provided by the proposed insured and the applicant at the time of application.



Remarks

(1)

The insured is eligible to receive a claim amount equal to 20% of the sum insured for a minor or juvenile illness during the coverage period. Multiple claims are allowed, but only once per specific illness. The maximum cumulative claim amount for these illnesses is 80% of the sum insured. Additionally, up to two claims can be made for carcinoma-in-situ (each for an occurrence involving a different insured organ). For a major illness or in the event of the insured’s death, the claim amount is 100% of the sum insured, and only one claim can be filed for either event.

(2)

The maximum cumulative claim amount for minor interventions for coronary artery diseases is MOP/HKD360,000; the maximum cumulative claim amount for carcinoma-in-situ is MOP/HKD360,000; the maximum cumulative claim amount for a minor or juvenile illness for each insured is MOP/HKD 240,000.

(3)

The claim amount for major illnesses is the sum insured, minus any indebtedness, any payable premium, and any payout(s) for minor illness and juvenile illness claims.

(4)

The claim amount for a minor or juvenile illness is 20% of the sum insured, minus any indebtedness and any payable premium. The future premiums will remain unchanged.

(5)

The coverage period for intellectual impairment due to illness and/or accidental injury begins when the insured reaches the age of four (4). For details, please refer to the policy issued by LFH Life.

(6)

The diagnosis of any juvenile illness must occur before the insured reaches the age of 18. For details, please refer to the policy issued by LFH Life.

(7)

The insured must have reached the age of six (6) when the illness is diagnosed. For details, please refer to the policy issued by LFH Life.

(8)

The coverage for the illness will end once the insured reaches the age of sixty-five (65). For details, please refer to the policy issued by LFH Life.

(9)

The insured must have reached the age of eighteen (18) when the illness is diagnosed. For details, please refer to the policy issued by LFH Life.

(10)

The policy will be terminated once the insured reaches the age of seventy (70).

(11)

The claim amount for the insured’s death that occurs in the coverage period is 100% of the sum insured, minus any indebtedness, any payable premium, and any paid claim amount(s).



Risk Disclosure Statement:

a.

Exchange rate and currency risk disclosure: you will be exposed to exchange rate risk if you pay your premiums in a foreign currency. The exchange rate may fluctuate from time to time due to various factors, possibly leading to financial losses on your part. The changes in the exchange rate could result in the subsequent premiums payable (if any) being higher than the initial premium. You should consider the exchange rate risk before deciding on the currency for premium payment.

b.

The Policy Owner shall pay the premium on time during the premium term. Failure to pay the required amount by the end of the grace period (if applicable) designated by LFH Life may result in lapse or termination of the policy. Nevertheless, it is subject to the automatic premium loan (if applicable) (LFH Life will advance the premium due from the non-forfeiture value as an automatic premium loan) and non-forfeiture provisions (if applicable). If the policy is terminated or lapsed due to non-payment of the premium, the Policy Owner may receive a surrender value less than the total premiums paid and lose the insurance protection provided by the policy.

c.

LFH Life will terminate the policy before the maturity date if any one of the following events occur:


(i)

The insured passes away;


(ii)

LFH Life approves the Policy Owner’s written request for surrender;


(iii)

The policy lapses after the end of the grace period;


(iv)

The non-forfeiture value is zero (if applicable);


(v)

The total amount of the benefits LFH Life has paid or is going to pay has reached the maximum benefit limit for all coverages under the policy (if applicable)

d.

The actual inflation rate may exceed expectations; therefore, the real value of the amount you receive may be lower.



Incontestability

The incontestability clause applies only to the Death Benefit under the policy. Except for non-payment of premiums or fraud, the validity of the policy shall become incontestable after it has been in force for two (2) years during the insured’s lifetime, starting from the policy issue date or the policy reinstatement date, whichever is later. To prevent misunderstanding, LFH Life reserves the right to void the entire policy according to the provisions related to fraud, misrepresentation or non-disclosure of material facts, or as otherwise permitted by the law, without being constrained by the incontestability clause.



Misstatement of Age and/or Gender

This policy is issued based on the age, gender or other factual details of the insured as specified in the policy schedule. Subject to LFH Life’s rights in the case of fraud, if the age, gender or other factual details of the insured have been misstated, the payable premiums and all benefits provided by this policy will be recalculated based on the premiums paid and the benefits entitled according to the correct age, gender and other factual details of the insured. If there is an overpayment of any premium resulting from the insured overstating their age or misstating their gender, LFH Life will refund the excess portion accordingly. LFH Life may exercise the absolute discretion to terminate the policy upon discovering the insured’s correct age, gender and/or other factual details that would have rendered them ineligible for the policy, and is solely liable for a refund of the paid premiums (without interest).



Fraud, Misrepresentation or Non-disclosure of Material Facts

The Policy Owner shall provide statements and answers that are completely truthful and accurate to the best of their knowledge and belief. In case of any fraud, misrepresentation, or non-disclosure of any material fact in the insurance application (if any) or the declaration upon which this policy is based, concerning any other matter affecting this policy or LFH Life’s risk, or in connection with the making of any claim under this policy, LFH Life has complete and absolute discretion to void this policy, and all claims hereunder shall be forfeited. Any premium(s) paid by the Policy Owner in these circumstances, except for cases involving fraud, will be refundable.



Cooling-off Period

The Policy Owner has the right, by providing written notice, to request the cancellation of the policy and a refund of any premium(s) paid (the premium(s) will be refunded in the currency of the policy or MOP/HKD based on the exchange rate provided on the day of the refund. To the extent permitted by law, LFH Life may deduct from the premium(s) to be refunded any fees directly incurred from underwriting, including but not limited to the fees for medical examinations and investigations). The Policy Owner shall sign the notice and ensure its direct receipt by LFH Life’s principal office at Alameda Dr. Carlos D'Assumpcao, No. 398, Edificio CNAC, 4º Andar, Macau within the cooling-off period, which is 21 calendar days from the delivery of the policy to the Policy Owner or their representative.



Policy Surrender

As long as no claims have been approved for any major illness, and the policy has a guaranteed cash value (if applicable) after the cooling-off period, the Policy Owner may, while the policy is in force, request the insurer to surrender the policy and receive the cash value (if any). Their right to a surrender request remains applicable after policy renewal at the end of the first (and the subsequent) contract terms. LFH Life reserves the right to postpone the payment of the surrender value (if any) for no more than six (6) months from the surrender date. The Policy Owner shall file the surrender request using the form designated by LFH Life and submit it to LFH Life’s office in Macao. Upon surrender and payment of the surrender value, the policy shall be terminated, and LFH Life shall have no further liability under this policy.



Premium Adjustment

The payable premium of the plan at the time of application and renewal is calculated based on, but not limited to, the following factors (if applicable): the sum insured, gender, entry age, smoking habit, premium payment term, underwriting class, risk type and residency, and is not guaranteed. LFH Life reserves the right to review and adjust the premium payable at any time, with factors leading to potential adjustments including, but not limited to, the discrepancies between experience and current expectations.
