1. Daily hospitalization cash benefit

If, while this Plan is in force, the Insured is confined on the recommendation of a Registered Medical Practitioner as a resident in-patient in a Hospital due to Disability, the Company will, upon receipt of satisfactory proof, pay to the Policyowner the Amount of Coverage as stated in the Policy Schedule for each Day of Confinement subject to the provisions, conditions and limitations contained hereinafter. The benefit shall be payable from the first Day of Confinement up to the date of discharge from the Hospital but not exceeding five hundred (500) days for any one Disability. The above confinement must qualify as a Reasonable and Customary Hospital Confinement for benefits to be payable by the Company.


Where the Insured is suffering from different Disability, under no circumstances will the Company pay more than the Amount of Coverage as stated in the Policy Schedule for each Day of Confinement.

 

However in the case where hospital confinement occurs in places other than Macau, Hong Kong, Beijing, Shanghai, Tianjin, Guangzhou, Shenzhen, Zhuhai, Foshan, Huizhou, Dongguan, Zhongshan, Jiangmen, Zhaoqing, Taiwan, South Korea, Singapore, Malaysia, Thailand, Japan, North America, Europe, Australia and New Zealand, the amount payable for each Day of Confinement will be limited to fifty (50) percent of the Amount of Coverage as shown in the Policy Schedule.


2. Extra 50% cash benefit of Intensive Care Unit

If the Insured is confined in an Intensive Care Unit and Daily Hospital Cash Benefit is payable, LFHL will additionally pay an Intensive Care Benefit equal to 50% of the amount of Daily Hospital Cash Benefit for each Day of Confinement in Intensive Care Unit up to forty-five (45) days for any one Disability.

3. Death Benefit

In the event of the death of the Insured while this Plan is in force and prior to the expiry date, the Company will, upon receipt of satisfactory proof of death, pay to the estate of the Insured the amount equal to one hundred and ten per cent (110%) of the total premium paid for this Plan as shown in the Policy Schedule less any Indebtedness and/or any due and unpaid premium in accordance with this Plan (if any).

4. Maturity Benefit

After the Maturity Date, the Company shall pay to the Policyowner the Maturity Benefit which shall be equal to one hundred and ten per cent (110%) of the total premium paid for this Plan as shown in the Policy Schedule less any Indebtedness and/or any due and unpaid premium in accordance with this Plan (if any) provided the all of the following requirements are met:

(i) this Policy is still in force on the Maturity Date;

(ii) the Insured is still alive on the Maturity Date; and

(iii) all premium due under this Policy up to and including the Maturity Date have been paid.

5. No medical examination required.

This plan specially offers a simple application for insurance, ensuring no medical examination is required and the application process is convenient and fast.


Except if the applicable law provides otherwise, this Plan shall not cover any claims caused directly or indirectly, wholly or partly, by any one of the following occurrences:


1. any hospitalization or charges incurred as a result of any condition or illness of the Insured which symptoms first manifest or occur or for which the Insured has received medical treatment or been attended to by a Physician or been prescribed drugs, in each case, during the five (5) years prior to or within thirty (30) days following the Issue Date, date of endorsement or date of any reinstatement of this Plan, whichever is later;


2. treatment or surgery for tonsils, adenoids, hernia or a disease peculiar to the female generative organs unless the Insured has been continuously covered under this Plan for a period of one hundred and twenty (120) days immediately after the Issue Date, date of endorsement or date of any reinstatement of this Plan (whichever is later) immediately preceding such treatment or surgery;


3. Pre-existing Condition or any complication arising therefrom;


4. suicide or self-inflicted injuries while sane or insane;


5. heredity condition, developmental condition, or any complications arising therefrom; congenital conditions existing at the time of birth (e.g. redundant prepuce, phimosis, harelip, flat foot etc.);


6. general health checks, routine physical examination, convalescence, custodial, rest care or genetic test, dental treatment, dentures, eye examination, glasses, hearing aids or the fitting thereof, corrective aids and treatment of refractive errors, cosmetic surgery or plastic surgery unless necessitated by Injury caused by an Accident occurring after the Issue Date, date of endorsement or date of any reinstatement of this Plan (whichever is later) and the Insured sustains the Injury and requires cosmetic surgery or plastic surgery within ninety (90) days of such Accident;


7. Human Immunodeficiency Virus (HIV) infection, except and to the extent that such infection occurs through an accidental cut or wound, and/or any HIV-related illness including AIDS and/or any mutations, derivations or variations thereof; venereal diseases, sexually transmitted diseases; mental or nervous diseases or disorder including but not restricted to anorexia nervosa, anxiety, depression, mania, neurosis, paranoid, psychosis, mental disorder, psychological disorder or psychiatric disorder, schizophrenia or personality disorder;


8. Drug-taking other than under the direction of a Physician, Injury or Sickness sustained whilst the Insured is under the influence of alcohol or drugs and treatment in connection with addiction to drugs or alcohol;


9. childbirth (including surgical delivery), pregnancy and complications thereof, miscarriage, abortion, infertility or sterilisation, pre-natal or post-natal care or conditions arising from surgical, mechanical or chemical contraceptive methods of birth control or treatment pertaining to infertility;


10. refractive errors of the eyes or their correction by glasses;


11. any treatment, investigation, services or supplies which are not Medically Necessary;


12. treatment of obesity (including morbid obesity), weight control programmes or bariatric surgery (except when bariatric surgery is necessary as confirmed by a Doctor after failure of conventional treatments and approved by the Company in advance);


13. confinement in an institution for extended care, or a place for alcoholics or drug addicts, or a nursing/rest/convalescent home, or rehabilitation centre, or home for the aged, or hydro-clinic or similar establishment;


14. sexual dysfunction including but not limited to impotence, erectile dysfunction or pre-mature ejaculation, regardless of cause;


15. war whether declared or undeclared or any act thereof, invasion, act of foreign enemies, hostilities, civil commotion, riots, rebellion, revolution, insurrection, military or usurped power or any warlike; and/or any medical malpractice occurred in the area where the above incidents was declared; or service in the armed forces in time of declared or undeclared war or while under orders for warlike operations or restoration of public order; or violation or attempted violation of the law or resisting arrest or participation in any brawl or affray;


16. engaging in or taking part in (a) driving or riding in any kind of race; (b) professional sports; (c) water and underwater activities involving the use of breathing apparatus; (d) aerial flights (including bungee-jumping, hang-gliding, ballooning, parachuting and sky-diving); (e) flying or other aerial activity except as a fare-paying passenger in a commercial aircraft;


17. driving any kind of vehicle while the alcohol level in the Insured's breath, blood, or urine is higher than the legal limit in the country or region where the driving takes place;


18. organ transplant services as a donor (except approved by the Company in advance);


19. Alternative treatments, including but not limited to Chinese medicine practitioners' conditioning, summer therapy for winter sickness, winter therapy for summer sickness, sanfutie therapy, moxibustion, massage, naprapathy, hypnosis, aromatherapy, natural therapy, speech therapy or nutritional therapy, etc.

The Policy Owner is subject to the credit risk of LFH Life. If the Policy Owner discontinues and/or surrenders the insurance plan during the early years of the policy, the benefit they receive may be considerably less than the amount of premiums they have paid.

 

LFH Life reserves the right to decide whether to accept or reject any application for the Plan and its optional benefits (if any) according to the information provided by the proposed insured and the applicant at the time of application.

 

 

Risk Disclosure Statement:

a. Exchange rate and currency risk disclosure: you will be exposed to exchange rate risk if you pay your premiums in a foreign currency. The exchange rate may fluctuate from time to time due to various factors, possibly leading to financial losses on your part. The changes in the exchange rate could result in the subsequent premiums payable (if any) being higher than the initial premium. You should consider the exchange rate risk before deciding on the currency for premium payment.

 

b. The Policy Owner shall pay the premium on time during the premium term. Failure to pay the required amount by the end of the grace period (if applicable) designated by LFH Life may result in lapse or termination of the policy. Nevertheless, it is subject to the automatic premium loan (if applicable) (LFH Life will advance the premium due from the non-forfeiture value as an automatic premium loan) and non-forfeiture provisions (if applicable). If the policy is terminated or lapsed due to non-payment of the premium, the Policy Owner may receive a surrender value less than the total premiums paid and lose the insurance protection provided by the policy.

 

c. LFH Life will terminate the policy before the maturity date if any one of the following events occur:

(i) The insured passes away;

(ii) LFH Life approves the Policy Owner’s written request for surrender;

(iii) The policy lapses after the end of the grace period;

(iv) The non-forfeiture value is zero (if applicable);

(v) The total amount of the benefits LFH Life has paid or is going to pay has reached the maximum benefit limit for all coverages under the policy (if applicable)

 

d. The actual inflation rate may exceed expectations; therefore, the real value of the amount you receive may be lower.

 

 

Incontestability:

The incontestability clause applies only to the Death Benefit under the policy. Except for non-payment of premiums or fraud, the validity of the policy shall become incontestable after it has been in force for two (2) years during the insured’s lifetime, starting from the policy issue date or the policy reinstatement date, whichever is later. To prevent misunderstanding, LFH Life reserves the right to void the entire policy according to the provisions related to fraud, misrepresentation or non-disclosure of material facts, or as otherwise permitted by the law, without being constrained by the incontestability clause.

 

Suicide:

If the insured commits suicide within one year from the date of issue or reinstatement of this policy (whichever is later), whether or not the insured was of sound mind at the time of suicide, the Company's liability shall be limited to the refund of the uninterest-bearing premiums paid, subject to deduction of any outstanding debts. If the policy has been reinstated, the refund of the premiums shall be calculated from the date of reinstatement.

 

Unless otherwise specified, these terms do not apply to any disability, accident or hospitalization coverage attached to this policy.

 

Misstatement of Age and/or Gender:

This policy is issued based on the age, gender or other factual details of the insured as specified in the policy schedule. Subject to LFH Life’s rights in the case of fraud, if the age, gender or other factual details of the insured have been misstated, the payable premiums and all benefits provided by this policy will be recalculated based on the premiums paid and the benefits entitled according to the correct age, gender and other factual details of the insured. If there is an overpayment of any premium resulting from the insured overstating their age or misstating their gender, LFH Life will refund the excess portion accordingly. LFH Life may exercise the absolute discretion to terminate the policy upon discovering the insured’s correct age, gender and/or other factual details that would have rendered them ineligible for the policy, and is solely liable for a refund of the paid premiums (without interest).

 

Fraud, Misrepresentation or Non-disclosure of Material Facts:

The Policy Owner shall provide statements and answers that are completely truthful and accurate to the best of their knowledge and belief. In case of any fraud, misrepresentation, or non-disclosure of any material fact in the insurance application (if any) or the declaration upon which this policy is based, concerning any other matter affecting this policy or LFH Life’s risk, or in connection with the making of any claim under this policy, LFH Life has complete and absolute discretion to void this policy, and all claims hereunder shall be forfeited. Any premium(s) paid by the Policy Owner in these circumstances, except for cases involving fraud, will be refundable.

 

Cooling-off Period:

The Policy Owner has the right, by providing written notice, to request the cancellation of the policy and a refund of any premium(s) paid (the premium(s) will be refunded in the currency of the policy or MOP/HKD based on the exchange rate provided on the day of the refund. To the extent permitted by law, LFH Life may deduct from the premium(s) to be refunded any fees directly incurred from underwriting, including but not limited to the fees for medical examinations and investigations). The Policy Owner shall sign the notice and ensure its direct receipt by LFH Life’s principal office at Alameda Dr. Carlos D'Assumpcao, No. 398, Edificio CNAC, 4º Andar, Macau within the cooling-off period, which is 21 calendar days from the delivery of the policy to the Policy Owner or their representative.

 

Policy Surrender:

As long as no claims have been approved for any major illness, and the policy has a guaranteed cash value (if applicable) after the cooling-off period, the Policy Owner may, while the policy is in force, request the insurer to surrender the policy and receive the cash value (if any). Their right to a surrender request remains applicable after policy renewal at the end of the first (and the subsequent) contract terms. LFH Life reserves the right to postpone the payment of the surrender value (if any) for no more than six (6) months from the surrender date. The Policy Owner shall file the surrender request using the form designated by LFH Life and submit it to LFH Life’s office in Macao. Upon surrender and payment of the surrender value, the policy shall be terminated, and LFH Life shall have no further liability under this policy.

 

Automatic Termination:

This Plan shall automatically terminate:

1. upon the death of the Insured; or

2. if and when this Plan matures, expires, is lapsed, or surrendered; or

3. on the Policy Anniversary on or immediately following the Insured's seventy (70) birthday.

 

 

Duplicate Policies:

The Insured shall not at any time be covered by more than one hospital cash plan unless the total sum of the Daily Hospital Cash Benefit as specified in the Policy Schedules of the relevant policies is less than one thousand eight hundred (1,800) Macau Patacas or Hong Kong Dollars. If there is more than one such policy issued to the Insured with total sum of the Daily Hospital Cash Benefit exceeding one thousand eight hundred (1,800) Macau Patacas or Hong Kong Dollars, the Company will pay the Daily Hospital Cash Benefit by following the sequence of policy issuance and in any event for each day of Hospital confinement the total Daily Hospital Cash Benefit payable under those policies will not exceed one thousand eight hundred (1,800) Macau Patacas or Hong Kong Dollars.

 

Notwithstanding anything to the contrary contained in any pre-existing policy(ies) of hospital cash plan issued to the Insured, this clause shall replace and supersede the provision of “Duplicate Policies” in any pre-existing policy(ies) of hospital cash plan issued to the Insured with immediate effect.
